Session Description for Review (limit 3,000 characters, including spaces)
Session Description for Publication (web, conference program) (limit 800 characters, including spaces)
Learning Objectives (limit 200 characters, including spaces)
Contact Information for the following individuals:

  1. Session Contact* (person submitting abstract and to receive official notification)
  2. Session Moderator (if known)
  3. Presenters (up to 3 additional presenters)

*Please note: if your session contact person is also your moderator and presenter, you will need to list that person in each area. If you do not have a moderator at this time, the Summit can help assign one to your session.

*Due to space limitations in the summit program, contributing authors will not be listed.   

You must create a user name and login to the abstract site to submit for the summit. Failure to complete the entire application will result in it not being reviewed.
